It was the date that the delivery of diabetes enhanced care became the responsibility of the Bradford Provider Alliance.As part of this, Bradford Care Alliance is contracting all of its current providers in Primary Care (these were previously the Diabetes Level 2 providers) to deliver this service as we transition. You will have received an email from Marie Stout asking for some key information for the contract.
