AS we develop a new model of primary care for the district, please remember to let us have your completed “community footprint†forms, detailing your group members and top priorities, by this Monday, October 2. Read on for more details about the next step on our journey to being part of an Accountable Care System.
PRIMARY care is experiencing lots of challenges. We at BCA along with the CCG would like to develop a new model of primary care.
This fits in with the NHS Five Year Forward View aim of an Accountable Care System.
The BCA is working in conjunction with the CCGs, Bradford Teaching Hospitals NHS Foundation Trust, Bradford District Care NHS Foundation Trust, voluntary care Sector and other providers in leading the next step on our journey to Accountable Care.
We have asked you to develop local footprints/communities of between 30,000 – 60,000 populations to map services around these groups.
The geography of these footprints will be determined by the GP practices based on who they would like to work with.

Details of future engagement events around these communities will be sent out to you in the near future.
